How can we help?
We have an open referral system, where carers can refer themselves for support by phoning or calling into the office. We also receive referrals from Social Services, Health professionals, Primary Care professionals and other Voluntary Agencies.
Each carer who is referred to the service is offered a home visit, which enables us to assess their needs and to provide them with the relevant information and refer them to appropriate agencies as needed.
The home visit allows us to:
find out whether the family are in receipt of the benefits that they are entitled to
To assess the impact that the caring role is having on the carer and if they require any extra practical support
To assess if the carer requires ongoing emotional support
Find out if the carer knows their rights as a carer to a Carers Assessment from Social Care & Health
Ensure the carer has all the information they require about the disability of the person they care for, the relevant services that are available in the Borough and how they can access them
