The Social Care & Health Linkworker Role was established to identify and support carers through partnership working with Social Care & Health (Social Services). The main objectives of the role include:
To increase awareness of the support needs of carers and work in partnership with professionals to provide such support.
Provide direct emotional support to help carers work through issues related to their caring role.
Provide practical support to improve the carers quality of life.
Work in partnership with key organisations to achieve positive outcomes for carers.
This service commenced at the beginning of October 2004, and was created from an identified need to increase referrals from Social Care and Health as carers who required support were not being referred to our service.
As with the Primary Care Service, a linkworker has been identified in each Social Care & Health team. Each linkworker is responsible for liaising with the Carers Centre to discuss identified carers issues, and to pass information to their team.
